1. Successfully complete 75 hours of training covering subjects specified in Health Care Financing Administration’s Conditions of Participation for Home Health Agencies, 42 CFR Part 484
2. Complete a minimum of 16 clinical hours
3. Receive at least 12 hours of in-service training annually
